Here at Ortiz Eye and Hearing Associates, we provide medical and routine care. Every comprehensive exam our doctors do includes looking at the health of your eyes, providing tests when necessary to help protect your eyes and vision, and providing a prescription at the end of your visit. Because of the extensive comprehensive exams, we only bill medically. We do not take any vision plans or bill any vision plans.
Payment & Insurance Policy
Ortiz Eye and Hearing Associates, unless told otherwise, will submit all medical charges to your medical benefits. This means that deductibles, co-insurance and co-pays will apply. For in-network plans, we will submit to insurance first, and whatever is not paid by your insurance, you, the patient, will receive a bill for. For all other charges, payment will be due the same day services are received.
For out-of-network plans, payment is due in full on the day of your appointment.
- Please be aware that we are out-of-network with all HMO plans. We also do not accept any HMO plans primary with a PPO plan secondary.
- Example: UHC HMO primary w/ BCBS PPO secondary. This also applies to HMO primary w/ Medicaid secondary.
We do give a 30% medical discount (vision services excluded) for all private pay patients with no insurance, out-of-network insurance, or high deductible insurance.

- Comprehensive Eye Exam, New Patient: $280.00. Refraction is always done, which is $38.00
- Comprehensive Eye Exam, Established Patient: $240.00. Refraction is always done, which is $38.00
Please be aware that the refraction portion of the exam is vision and not covered by medical insurance, that charge will be out of pocket, and patients can turn a receipt into a vision plan themselves.
Also, depending on what our doctors find at your exam, there could be additional testing that's done, which will be additional charges added on your invoice.
Ortiz Eye and Hearing Associates is dedicated to working with you to help you with processing your out-of-network benefits. We understand how hard it is to find out that your vision coverage won’t help you with an eye exam or get the frames you really want and need. Many patients utilize the reimbursement
